Bridges Self Management – Team Workshop, up to 10 people Apply Bridges Self Management – Team Workshop, up to 10 people Bridges Self Management strongly advocate a ‘whole systems’ approach to self-management support by considering how teams work, the service they provide and the needs of individual clients and their families. We support teams to challenge and enhance their practice and arrive at a shared understanding of what effective self-management support looks like in their particular context. It is possible to come to the training if you work with any caseload of individuals with acute or long term conditions as we feel that the principles of self-management support can be universally applied. What is the aim of team training? To enable a multidisciplinary team to build, evaluate and sustain a shared approach to supporting self-management, reflecting a shift towards successful person-centred care. Access to unique self-management too – Where applicable to your caseload, you will be supplied with our unique self-management tools for either stroke, TBI or long-term conditions, to use with patients and families What are the potential benefits for our service? – Teams who work in a targeted person-centred way can improve the quality of their interactions with patients and families, which in turn can lead to improvements in measures of patient experience and satisfaction. Secondary service outcomes that can be expected include more efficient use of health and social care resources. What are the potential benefits for our patients and their families? – Patients and families who feel supported to self-manage feel greater confidence and control over their condition and daily lives. In turn this can bring about improvements in mood, functional ability, and perceived quality of life. What do other practitioners say? – Bridges gives them the language, structure and evidence-base to enhance what they already do. – The team feels more confident to discharge people from their services and they are less likely to refer people on to other services – The team has a more consistent, person-centred approach to practice, with a focus on what’s important to the person and their family. Learning outcomes: Understand the key policies and research in relation to supporting living with a long-term condition Appreciate the enablers and barriers to effective self-management in the long-term Understand the theoretical and evidence-base to the Bridges approach to self-management support Demonstrate practical methods of enhancing self-efficacy in individuals, thereby achieving greater control in their everyday lives Demonstrate greater awareness of different communication styles and their impact on supporting self-management Demonstrate ways of using the unique self-management tools with patients and families and across different clinical settings Understand ways of involving family, friends and carers in supporting self-management Develop strategies to evaluate and sustain self-management support in practice. The Bridges team will guide you in ways to implement and evaluate self-management support strategies; Bridges is ideal as a service improvement project to test an innovative way to integrate self-management support into rehabilitation and care Who is the module for? Frontline staff in health and social care of any grade working in the same team, service or pathway. Module code: BRDG004A Location: At your place of work, or a local venue suitable for your team Pattern of Delivery: Workshops are delivered at your place of work or a local venue suitable to the team, by two Bridges trainers. Bridges trainers often work alongside Bridges associate trainers who represent the multidisciplinary team and share their own experiences of successfully implementing Bridges in practice. Two stage training – full day initial workshop (9am – 4.30pm) and half-day follow up workshop (9am – 1pm) approx. 3 months later.
